Information included on the form will be used by the sports information department to highlight your accomplishments and complete your bio on the athletics website. By filling out the form, you give OPSU Athletics the right to publish pertinent information to the website and all affiliated social media channels, as well as the right to disseminate the information to media as the sports information department sees fit.
